This Privacy Policy explains how SurveyForm handles personal information when people visit the website, create an account, build surveys, submit survey responses, or contact us. It is intended to be read together with the Terms of Use.
SurveyForm is designed for Australian users and is hosted on servers located in Australia.
2. Personal information we collect
The information we collect depends on how you use the service. It may include:
- account details, such as name, email address, encrypted password, role, account status, and creation date;
- survey content, including questions, descriptions, settings, uploaded images, and public survey links;
- survey response data, including answers submitted by respondents, respondent email where provided, submission time, and IP address;
- technical information, such as session cookies, device/browser information, server logs, security events, and usage activity;
- communications you send to the site owner, administrator, or support provider.
3. How information is collected
We collect information when users create accounts, update settings, build surveys, upload content, publish surveys, submit responses, export data, sign in, or otherwise interact with the service. Some technical information is collected automatically to keep the service secure and functioning.
Survey owners decide what questions to ask respondents. If you are a respondent, the survey owner is responsible for explaining the purpose of their survey and how they will use exported response data.
4. How we use information
We use personal information to:
- provide, secure, maintain, and improve SurveyForm;
- create and manage user accounts and access permissions;
- publish surveys, collect responses, display results, and generate exports;
- support account administration, troubleshooting, audit, and fraud prevention;

5. Cookies and sessions
SurveyForm uses cookies and server-side sessions to keep users signed in, protect forms from misuse, remember temporary messages, and support account security. Session cookies are not used for third-party advertising.

- to account owners, administrators, and authorised users within the relevant workspace;
- to survey owners where a respondent submits a response to that owner's survey;
- to hosting, infrastructure, email, security, analytics, or support providers that help operate the service;

- where necessary to protect rights, safety, security, or the integrity of the service.
We do not sell personal information.
7. Overseas disclosure
The service is hosted in Australia. Some support, software, security, or communications providers may process limited information from outside Australia depending on the tools used to operate the service. Where this occurs, we take reasonable steps to use reputable providers and protect the information.
8. Security
We use reasonable technical and organisational safeguards to protect information, including access controls, secure password hashing, session protections, database controls, and restrictions on direct access to sensitive server folders. No online service can guarantee absolute security.

Survey owners are responsible for managing the retention and deletion of survey content and response exports they control.
10. Access and correction
Account users can update certain account details after signing in. You may request access to, or correction of, personal information held about you by contacting the site owner or administrator. Respondents should generally contact the survey owner first, because the survey owner controls the purpose and use of their survey responses.
11. Complaints
If you have a privacy concern, contact the site owner or administrator with details of the issue. We will aim to review and respond within a reasonable time. If you are not satisfied, you may be able to contact the Office of the Australian Information Commissioner or another relevant privacy regulator.
12. Changes to this policy
We may update this Privacy Policy when our practices, technology, legal obligations, or service features change. The updated version will be posted on this page with a new last updated date.
